Seniors will be completing a "Senior Check-Out Form" prior to graduation. Students will indicate where to send their final transcript. Final transcripts will be submitted electronically through Naviance or mailed to the appropriate colleges no later than June 30th of each school year.

• CURRENT students requesting a transcript must bring their Student ID into the Registrar's office. Transcripts for National Honor Society can be printed from student's Aeries account.
• SENIORS requesting a transcript for a college application MUST see their counselor.
• ATHLETES requesting a transcript for NCAA must bring their Student ID into the Registrar's office. 

If you are a former student of Carlsbad High School and are requesting an Official Transcript, email your request to ddagy@carlsbadusd.net

1. First and Last Name (used at CHS)
2. Date of Birth
3. Graduation Year (or last year attended)
4. Delivery preference (mailed or picked up)
5. If mailed, provide complete address
